    Vibration Alert Bracelet for Notification of the Visually and Hearing Impaired

    This paper presents the prototype of an electronic vibration bracelet designed to help the visually and hearing impaired to receive and send emergency alerts. The bracelet has two basic functions. The first function is to receive a wireless signal and respond with a vibration to alert the user. The second function is implemented by pushing one button of the bracelet to send an emergency signal. We report testing on a prototype system formed by a mobile application and two bracelets. The bracelets and the application form a complete system intended to be used in retirement apartment communities. However, the system is flexible and could be expanded to add new features or to serve as a research platform for gait analysis and location services. The medical and professional potential of the proposed system is that it offers a simple, modular, and cost-effective alternative to all the existing medical devices with similar functionality currently on the market. The proposed system has an educational potential as well: it can be used as a starting point for capstone projects and demonstration purposes in schools to attract students to STEM disciplines

    Tendon-Based Transmission for Surgical Robotics: Systematic Experimental Friction Modeling.

    Increased miniaturization of surgical instruments is essential to successfully perform surgical procedures in restricted areas as in many applications of minimally-invasive surgery. Miniaturization permits increase in dexterity and decrease in access incisions which are required in many sur- gical procedures. Tendon-based transmissions provide several important advantages for the mechanical design of miniaturized surgical devices. Reļ¬‚ected mass and inertia are reduced since tendon-based transmissions allow to locate the motors far apart from the actuated joint. In spite of providing several important advantages, they introduce several non linear effects that must be considered and modeled to achieve suitable performance. In this paper tendon-based transmission system for surgical robotics is illustrated. And nonlinear friction, due to direct sliding of synthetic ļ¬ber cables over small ļ¬xed pulleys (pins) is discussed and models aiming at compensating these nonlinearities are presented

    A typology of fisheries management tools: using experience to catalyse greater success

    Fisheries provide nutrition and livelihoods for coastal populations, but many fisheries are fully or over-exploited and we lack an approach for analysing which factors affect management tool performance. We conducted a literature review of 390 studies to assess how fisheries characteristics affected management tool performance across both small-scale and large-scale fisheries. We defined success as increased or maintained abundance or biomass, reductions in fishing mortality or improvements in population status. Because the literature only covered a narrow set of biological factors, we also conducted an expert elicitation to create a typology of broader fishery characteristics, enabling conditions and design considerations that affect performance. The literature suggested that the most commonly used management tool in a region was often the most successful, although the scale of success varied. Management tools were more often deemed successful when used in combination, particularly pairings of tools that controlled fishing mortality or effort with spatial management. Examples of successful combinations were the use of catch limits with quotas and limited entry, and marine protected areas with effort restrictions. The most common factors associated with inadequate biological performance were ā€˜structuralā€™ issues, including poor design or implementation. The expert-derived typologies revealed strong local leadership, high community involvement and governance capacity as common factors of success across management tool categories (i.e. input, output and technical measures), but the degree of importance varied.     Development of maternal seed tissue in barley is mediated by regulated cell expansion and cell disintegration and coordinated with endosperm growth

    After fertilization, filial grain organs are surrounded by the maternal nucellus embedded within the integuments and pericarp. Rapid early endosperm growth must be coordinated with maternal tissue development. Parameters of maternal tissue growth and development were analysed during early endosperm formation. In the pericarp, cell proliferation is accomplished around the time of fertilization, followed by cell elongation predominantly in longitudinal directions. The rapid cell expansion coincides with endosperm cellularization. Distribution of TUNEL (terminal deoxynucleotidyl transferase-mediated dUTP nick end labelling)-positive nuclei reveals distinct patterns starting in the nucellus at anthesis and followed later by the inner cell rows of the pericarp, then spreading to the whole pericarp. The pattern suggests timely and spatially regulated programmed cell death (PCD) processes in maternal seed tissues. When the endosperm is coenocytic, PCD events are only observed within the nucellus. Thereby, remobilization of nucellar storage compounds by PCD could nourish the early developing endosperm when functional interconnections are absent between maternal and filial seed organs. Specific proteases promote PCD events. Characterization of the barley vacuolar processing enzyme (VPE) gene family identified seven gene members specifically expressed in the developing grain. HvVPE2a (known as nucellain) together with closely similar HvVPE2b and HvVPE2d might be involved in nucellar PCD. HvVPE4 is strongly cell specific for pericarp parenchyma. Correlative evidence suggests that HvVPE4 plays a role in PCD events in the pericarp. Possible functions of PCD in the maternal tissues imply a potential nutritive role or the relief of a physical restraint for endosperm growth. PCD could also activate post-phloem transport functions

    A prospective multicenter validation study for a novel angiography-derived physiological assessment software:Rationale and design of the radiographic imaging validation and evaluation for Angio-iFR (ReVEAL iFR) study

    Background Angiography-derived physiological assessment of coronary lesions has emerged as an alternative to wire based assessment aiming at less-invasiveness and shorter procedural time as well as cost effectiveness in physiology-guided decision making. However, current available image-derived physiology software have limitations including the requirement of multiple projections and are time consuming. Methods/Design The ReVEAL iFR (Radiographic imaging Validation and EvALuation for Angio-iFR) trial is a multicenter, multicontinental, validation study which aims to validate the diagnostic accuracy of the Angio-iFR medical software device (Philips, San Diego, US) in patients undergoing angiography for Chronic Coronary Syndrome (CCS). The Angio-iFR will enable operators to predict both the iFR and FFR value within a few seconds from a single projection of cine angiography by using a lumped parameter fluid dynamics model. Approximately 440 patients with at least one de-novo 40% to 90% stenosis by visual angiographic assessment will be enrolled in the study. The primary endpoint is the sensitivity and specificity of the iFR and FFR for a given lesion compared to the corresponding invasive measures. The enrollment started in August 2019, and was completed in March 2021. Summary The Angio-iFR system has the potential of simplifying physiological evaluation of coronary stenosis compared with available systems, providing estimates of both FFR and iFR. The ReVEAL iFR study will investigate the predictive performance of the novel Angio-iFR software in CCS patients. Ultimately, based on its unique characteristics, the Angio-iFR system may contribute to improve adoption of functional coronary assessment and the workflow in the catheter laboratory

    Application of cerium chloride to improve the acid resistance of dentine

    OBJECTIVE: To investigate the effect of cerium chloride, cerium chloride/fluoride and fluoride application on calcium release during erosion of treated dentine. METHODS: Forty dentine samples were prepared from human premolars and randomly assigned to four groups (1-4). Samples were treated twice a day for 5 days, 30s each, with the following solutions: group 1 placebo, group 2 fluoride (Elmex fluid), group 3 cerium chloride and group 4 combined fluoride and cerium chloride. For the determination of acid resistance, the samples were consecutively eroded six times for 5 min with lactic acid (pH 3.0) and the calcium release in the acid was determined. Furthermore, six additional samples per group were prepared and used for EDS analysis. SEM pictures of these samples of each group were also captured. RESULTS: Samples of group 1 presented the highest calcium release when compared with the samples of groups 2-4. The highest acid resistance was observed for group 2. Calcium release in group 3 was similar to that of group 4 for the first two erosive attacks, after which calcium release in group 4 was lower than that of group 3. Generally, the SEM pictures showed a surface coating for groups 2-4. No deposits were observed in group 1. CONCLUSION: Although fluoride showed the best protective effect, cerium chloride was also able to reduce the acid susceptibility of dentine significantly, which merits further investigation

    Characteristics of dietary intake in relation to the consumption of home-produced foods among farm women in two rural areas of Kenya: A preliminary study

    The present study aimed to clarify the differences in nutritional intake in relation to the consumption of local food products and dietary patterns between two rural Kenyan regions, Kitui and Vihiga, where different ethnic groups live in different agro-ecological zones. A participant observation study with weighted dietary records was conducted in August 2019. Enumerators stayed in each targeted household for approximately one week and measured the ingredients and dishes. We compared the dietary intake of farm women in charge of meal preparation (n = 21) between the two regions and examined the contribution of each dish to the intake and the degree of home production for each food item. The results showed no difference in energy intake, but vitamin B2, B12, and C intakes were significantly higher in Vihiga, influenced by their consuming small fish and a variety of homegrown leafy vegetables. The people in Kitui consumed large quantities of homegrown pigeon peas, largely contributing to their nutritional intake. Dietary patterns were similar; common staple foods and tea with sugar accounted for about 40% of energy and protein intakes and fruit consumption was low. There was no difference in foods purchased frequently. These results suggested that promoting locally available fruits and vegetables would contribute to a sustainable supply of adequate micronutrients. Further studies are required to develop strategies to promote healthy dietary habits and improve health status

    Ticagrelor monotherapy in patients with concomitant diabetes mellitus and chronic kidney disease: a post hoc analysis of the GLOBAL LEADERS trial.

    BACKGROUND Patients with both diabetes mellitus (DM) and chronic kidney disease (CKD) are a subpopulation characterized by ultrahigh ischemic and bleeding risk after percutaneous coronary intervention. There are limited data on the impact of ticagrelor monotherapy among these patients. METHODS In this post hoc analysis of the GLOBAL-LEADERS trial, the treatment effects of the experimental (one-month dual-antiplatelet therapy [DAPT] followed by 23-month ticagrelor monotherapy) versus the reference regimen (12-month DAPT followed by 12-month aspirin alone) were analyzed according to DM/CKD status. The primary endpoint was a composite endpoint of all-cause death or new Q-wave myocardial infarction at 2-years. The patient-oriented composite endpoint (POCE) was defined as the composite of all-cause death, any stroke, site-reported MI and any revascularization, whereas net adverse clinical events (NACE) combined POCE with BARC type 3 or 5 bleeding events. RESULTS At 2 years, the DMā€‰+ā€‰/CKDā€‰+ā€‰patients had significantly higher incidences of the primary endpoint (9.5% versus 3.1%, adjusted HR 2.16; 95% CI [1.66-2.80], pā€‰<ā€‰0.001), BARC type 3 or 5 bleeding events, stroke, site-reported myocardial infraction, all revascularization, POCE, and NACE, compared with the DM-/CKD- patients. Among the DMā€‰+ā€‰/CKDā€‰+ā€‰patients, after adjustment, there were no significant differences in the primary endpoints between the experimental and reference regimen; however, the experimental regimen was associated with lower rates of POCE (20.6% versus 25.9%, HR 0.74; 95% CI [0.55-0.99], pā€‰=ā€‰0.043, pinteractionā€‰=ā€‰0.155) and NACE (22.7% versus 28.3%, HR 0.75; 95% CI [0.56-0.99], pā€‰=ā€‰0.044, pinteractionā€‰=ā€‰0.310), which was mainly driven by a lower rate of all revascularization, as compared with the reference regimen. The landmark analysis showed that while the experimental and reference regimen had similar rates of all the clinical endpoints during the first year, the experimental regimen was associated with significantly lower rates of POCE (5.8% versus 11.0%, HR 0.49; 95% CI [0.29-0.82], pā€‰=ā€‰0.007, pinteractionā€‰=ā€‰0.040) and NACE (5.8% versus 11.2%, HR 0.48; 95% CI [0.29-0.82], pā€‰=ā€‰0.007, pinteractionā€‰=ā€‰0.013) in the second year. CONCLUSION Among patients with both DM and CKD, ticagrelor monotherapy was not associated with lower rates of all-cause death or new Q-wave, or major bleeding complications; however, it was associated with lower rates of POCE and NACE.
